Create Your Health Record

To create a PHR report you need to read the following instructions or view the pdf file.

Step01:

After logging to your account, choose the PHR link

Click to add personal information

*Step03:*
Save your data by clicking the submit button

*Step04:*
Now choose personal information from the navigational bar on the left side of your screen

*Step05:*
Add your Vital Signs

*Step06:*
Save the data you entered by clicking the Save button.

*Step07:*
You can now view your Vital signs history.

*Step08:*
Add clinic visits for Non-Ojus Consultations

*Step09:*
Save this data by clicking on the Save button

*Step10:*
View History of Non-Ojus Consultations

*Step11:*
Click the allergies link.

*Step12:*
View the in-built Database of Allergies

*Step13:*
You will not get to see your allergy details

*Step14:*
Click the immunization link

*Step15:*
enter your immunization details

*Step16:*
view your immunization details

*Step17:*
Click the insurance link on the navigation bar on the left hand side

*Step18:*
Enter your insurance policy/details and click save to store them

*Step19:*
Upload documents from labs, radiology centers etc.

*Step20:*
View reports.

*Step21:*
View your detailed personal health record once you have filled up all the required details,You can always go back to add more information if required

*Step22:*
Choose the Doctor review link to see which reports of yours have been checked by the doctor.
