| 1)Physician's reports (also known as EMR Reports) |

Online reports generated by Ojus or Ojus-empanelled physicians after providing consultation at any of our clinics or over phone/video conferencing. |

2)Personal Health Records (PHR) |
| |
Members can create Personal Health Records for self and family members. PHRs are used to capture data from:a)Health data such as a blood sugar or BP obtained from periodic tests done at home. Data can be shared with Ojus physician for analysis
b)Any data from consultations/treatment you have procured at other health centres. For ease of use, reports and other documents can also be scanned and attached for convenience.

General Terms and Notes: |

1) The Ojus member ID card covers the Railwire customer (primary member) and immediate family members (Total of 6 persons). The membership fee for this program is Rs. 100 per month for a family of 6. Additional family members can be added to the program at an extra cost of Rs. 25 per person month.

2) There is a free trial offered to Railwire customers till August 31, 2011. The above mentioned membership fee has been waived during this time. The free trial includes waiver of only Ojus' membership fee. Services will be charged as applicable as per standard rates of Ojus and its partners. |

3) Only the primary member's ID is displayed on the card. For family members also to avail of the services, the primary member must add each member to his his/her account. In the process, separate Ojus membership ID numbers will be generated for each family member. Members not shown on the primary member's account will not be considered as a part of the program.

4) Whenever members wish to procure healthcare from health centres of Ojus or any of its partners, they must present their Ojus ID card AND mention the ID of the person receiving the care as well.

5) In order to obtain telephone based services, you will need to mention the member ID number of the patient to Ojus' Medical Helpline executive attending the call.
Caution: Always provide the member ID number of the person actually requiring the service. If a wrong ID number is given, it may be possible that clinical data and medical reports of the actual patient will be captured another persons' account. This can affect future consultations as doctors often rely on past history of each person when making a diagnosis.
